What if I disagree with whom the VA appointed as my fiduciary?
If you disagree with whom the VA appointed as your fiduciary you may submit a statement detailing why you disagree to your Fiduciary Hub of jurisdiction. This statement is called a Notice of Disagreement (NOD) and must contain your signature.
You may use VA Form 21-4138, Statement in Support of Claim, to submit your NOD. VA Form 21-4138 can found at:

The address to your Fiduciary Hub of jurisdiction may be found by logging onto http://www.vba.va.gov/bln/21/Fiduciary/contact.htm .
